Parental Permission for Border Crossing

into Mexico

IMPORTANT NOTE: Your minor youth will not be permitted to cross the USA border into Mexico without having this form completed and signed by both parents and/or legal guardians responsible for the minor child.

IN ADDITION: This year, if your minor youth is a citizen of the USA, he or she must have on their person a valid photo ID card and an official US Passport! If he or she is not a USA citizen, a valid passport and visa papers are required from his/her nation of origin. If you have questions, ask!

As parent/s and/or legal guardians of the following minor child/ren, we give our permission for the staff and clergy of St. George’s Orthodox Church and PRO-ORB Youth Retreat to transport across the USA border into Mexico and returning back to the United States within the retreat and service project period, which is scheduled for:
